MECHANISM OF ACTION GEPAMIN

The severe defeat of a hepatic parenchyma arising at chronic diffusion diseases of a liver as a result of direct extensive surgical interventions (a resection, transplantation) or owing to secondary damage of body at various diseases, injuries, septic states, is followed by standard disturbances from which leaders are: albuminolysis intensification, disorder of protein and amino-acid exchange, loss of immunity, ammoniac poisoning of an organism, disturbance of a functional condition of intestines and psychoneurological status.

Amino acids with a branched side chain (ABC) (a leucine, an isoleucine, valine) are the major participants gluco-ketogenesis. As a result of an albuminolysis processes intensification, from the muscles of amino acids which are released owing to disintegration the amino group is chipped off, increasing concentration of ammonia in an organism, and the carbon skeleton is oxidized on a glucoclastic way. In critical situations of ABC can cover up to 40% of energy consumption that leads to significant decrease in their concentration in out of - and intracellular spaces.

Along with it, the developing dysfunctions of hepatic cells lead to change of concentration of a number of the amino acids which are initially metabolized in a liver. In particular, there is an increase in maintenance of aromatic (AA) (phenylalanine, tyrosine, tryptophan), sulfur-containing (methionine, cysteine), dicarbonic (aspartic, glutamate) amino acids and also decrease in concentration of such important amino acids as arginine and its exchanging ornithine analog. Since arginine and its analog ornithine  take part in a cycle of an ureapoiesis in which excess of ammonium is utilized, decrease in their concentration leads to increase in poisoning of an organism. Besides, it is known that arginine stimulates synthesis of albumin, immunoglobulins and phagocyte activity of neutrophils, so, it is capable to regulate humoral and cellular immunity.

At these states in an organism ammonium content significantly increases that in turn stimulates development of a glucagon - the major catabolic hormone promoting processes of an albuminolysis. Thus the mechanism of a vicious circle supporting disturbance of the balanced ratio of amino acids in a blood plasma works.

Unbalanced content of amino acids, along with other reasons, limits processes of formation of protein, contributes to the development of protein insufficiency and an immunodeficiency.

ABC and AA have identical рН and the uniform carrier, competing among themselves for passing through a blood-brain barrier. Decrease in a molar ratio of ABC/AA can lead to disturbance of transport of these amino acids through a blood-brain barrier, to increase in concentration of AA in tissue of a brain and cerebrospinal liquid, leading to possible pathological change of exchange of neurotransmitters (transmitters of nervous excitement) which predecessors are AA, and to development of disorders of the psychological status.

Along with the strengthened albuminolysis and decline in the ability of a liver to its education at these morbid conditions disturbance of functions of intestines takes great interest. Indications to Gepamin's use:

-      rehabilitations of patients with "a high metabolic stress" (extensive injuries and operations, sepsis, massive bleedings) with signs of disturbance of protein and amino-acid exchange, intestinal insufficiency;

-      rehabilitation of the patients suffering from an alcoholism;

-      disbyosis;

-      obstructions.

Gepamin application instruction:

The daily dose is calculated proceeding from total demand for amino acids with a branched side chain which makes 0.25-0.30 on body weight kg. The average dose of drug makes 2-6 packages or 6-18 tablets a day counting on the patient weighing 70 kg. 1 - 2 packages or 2-6 pill of drug take in time or directly after a meal, with water or fruit juice.

Duration of a course of therapy varies depending on the nature of pathology, weight and dynamics of a current of the main clinical syndromes of a disease. The course of treatment averages 2-4 weeks. If necessary courses of therapy can be prolonged or repeated before improvement of clinical symptomatology.
